In the following video, Akanksha Mehta, MD, MS, discusses updated recommendations from the 2026 American Urological Association (AUA) Vasectomy Guideline on vas isolation and occlusion techniques.1,2 Mehta is a professor of urology at Emory School of Medicine in Atlanta, Georgia.
The guideline places renewed emphasis on technique selection, particularly in the context of a highly variable vasectomy literature. According to Mehta, the panel carefully evaluated a wide range of procedural approaches, from traditional to minimally invasive methods, while prioritizing outcomes such as vasectomy failure and postoperative complications. As a result, the guideline issues a moderate recommendation, supported by Grade A evidence, for using a minimally invasive vas isolation approach, such as the no-scalpel vasectomy (NSV) technique.
For vas occlusion, the updated guideline strengthens prior recommendations by identifying the combination of mucosal cautery and fascial interposition as the optimal strategy. While mucosal cautery was already recommended in earlier iterations, the evidence base supporting fascial interposition has matured, leading to its elevation to a strong recommendation. Mehta emphasized that when both techniques are used together, urologists can achieve the lowest risk of vasectomy failure. In contrast, approaches such as segmental excision or the use of sutures and clips are acceptable, but without fascial interposition may be associated with higher failure rates.
Overall, the 2026 update underscores a shift toward evidence-based standardization, with a clear focus on techniques that maximize procedural success while maintaining efficiency and safety.
